failed to provide necessary information to enable him to replicate the calculation of his AVC benefit;

provided misleading and inaccurate information in its 2017 Annual Report as to when the triennial valuation was due; and

did not respond to his letter dated 7 November 2017 or to his e-mail dated 30 November 2017.
