Financial Assistance Scheme: interested parties form
If you want to participate in a FAS appeal you must apply to become a party to the complaint.
Please use the downloadable pdf form. Then email CentralSupportMailbox@pensions-ombudsman.org.uk with a copy of the completed form, along with copies of your complaint correspondence, including your complaint letter to the party/parties and their response (if received) as we cannot progress your complaint without this supporting information.
